It was wonderful to be able to highlight the work of our DELIVER team at the Showcase event in Warrnambool in November 2023. Thanks to Western Alliance for the opportunity to present as part of their annual Symposium.

The Showcase also included a panel discussion facilitated by Craig Wilding (Chief Operating Officer, Community and Aged Care and Allied Health, Grampians Health) around the theme of Virtual Monitoring: funding, equity and sustainability.
Following the DELIVER Showcase, the DELIVER team re-convened at the Deakin Warrnambool campus for an All Investigator Day.
The goal of this meeting was to discuss how to advance activities in 2024 under the aim of ‘optimising health care at home for older people’.
Teams shared the ideas arising from the initial exploration of factors impacting implementation of remote patient monitoring and uptake of home based care services for older people across the Barwon South West and Grampians regions.
These ideas are being compiled in a report that will be shared with our partners, and together we will make decisions about what research-informed strategies can be tested for feasibility.
